All student-athletes are required to adhere to policies and procedures outlined in the student-athlete handbook. All athletes will sign an acknowledgement form that they read and understand the policies outlined.

Pre-Season Forms

All student-athletes are required to complete necessary paperwork prior to the start of their season. Forms can be found in the FrontRush roster portal.  If you are getting a physical conducted by your own doctor, please use the form below. If you do not have insurance, please get the affadavit of no insurance notarize

1. Open link in email to front rush to create password.
2.  Create your password by following the prompt                                                                               
3. Login in with your email and new password
4.  Select “Forms” along the options under your name at the top of the page
5. Select a form to complete the form by carefully reading and filling in each box correctly
6. Select “Submit” (*Note: you will not be able to submit the form unless each box with a red asterisk is filled out)
7.  Repeat steps 5 and 6 for the rest of the forms in the list.
8.  The “Physical Examination Form” must be uploaded and a copy must be brought to campus with you if you did not receive a physical with the athletic training staff on campus.
